If you would like to know whether we have coverage in your area and would like us to check your benefits, please email stella@cta-pt.com the following:

  • Pictures of the front and back of your insurance card;

  • Child's full name and date of birth;

  • Policyholder's full name and date of birth;

  • Services needed (PT, OT, Speech, or any combination)

  • Your name, address, and phone number.
